Speaker 1: So 80% or so of my practice is gender affirming surgery. So I do facial chest surgery and genital surgery. But the majority of my practice instead of where my Passionalyzes, grossly genital surgery cases. So I do a lot of vaginoplasty and a lot of phalloplasty. I would just say they're Expanding in either direction. So, yeah, a lot of adolescents, presenting for Surgical intervention, but also a lot of people that are like in their seventies sometimes coming in for general surgery and then everything in between. But the the adolescents for sure present some unique challenges. Obviously there's great evidence supporting pubertal suppression for a whole variety of Methods. But the one thing that is very new is genital surgery in someone that has underwent puberal suppression. Not so much an issue in, someone with assigned female at birth anatomy that undergoes a phalloplasty, because we're creating something with, You know, free tissue transfer or flap anyway, but a much bigger issue for an individual that's undergoing a penile inversion, vaginoplasty. Because we use all of that tissue to basically create vulva as well as line the internal vaginal canal. And as a specialty, those of us that do a fairly high volume of general gender affirming surgery, you know, we've maybe done a couple, a handful of puberantly suppressed Adolescence as a field and no one's published on it yet. Oh, it just you is we're just putting our 1st series together as we're kind of learning and figuring out what works. But it's really changing things, because you don't have enough tissue to line the vaginal canal. So you either have To take a skin graft or take skin from elsewhere or use some artificial products. The way that we're dealing with it is by using a robot And we're basically performing intra abdominal, components of the surgery. So we're using peritoneum, which is the inner lining of the abdomen to line most of the vaginal canal. And by doing that, that allows us to use all of the remaining tissue externally to create a vulva, and try to make also an aesthetic result. Speaker 0: So I'm going to speak the truth here plainly and I think the vast majority of the majority of the American people understand and agree with what I'm going to say. What you've just heard there is a little sample of barbarism. This is the mutilation of children, and it should be prohibited by our law. This so called gender affirming care is anything but affirming and caring, and the language matters. This is adults deciding to permanently alter the bodies of children who do not have the capacity to make like life altering decisions on their own.

We need to work together on this, but we we owe it to the people people to know what the plan is, where the money is gonna be spent, and we need some auditing for the dollars that we've already sent over there. These are not tough questions. Right? One thing that House Republicans are resolved on is that we must stand with our most important ally in the Middle East, and that's Israel. We will. We certainly hope that it doesn't come to boots on the ground. If if it comes to that, and we communicated this to the White House staff as well today, that, you know, we have the article one power in the legislative branch of government, and they have article do a very limited authority on what they can do to respond without coming to congress to seek consent. Speaker 0: Thank you all for being here. Just had a good meeting with president Zelensky. I reiterated to him that we stand with him and against Putin's brutal invasion. The American people stand for freedom and they're on the right side of this fight. I have asked the White House since the day that I was handed the gavel as speaker For clarity, we need a clear articulation of the strategy to allow Ukraine to win. And thus far, their responses have been insufficient. They have not Provided us the clarity and the detail that we requested over and over since literally 24 hours after I was handed the gavel as speaker of the house. And so What the Biden administration seems to be asking for is 1,000,000,000 of additional dollars with no appropriate oversight, no clear strategy to win, And and none of the answers that I think the American people are owed. I have also made very clear from day 1 that our first condition on any national security supplemental spending package is about our own national security first. The border is an absolute catastrophe, and this is because of the policies of this White House and this administration. We had 12,000 illegal crossings on one day last week alone, on Wednesday. We have, Almost now 280 known terrorists that have been apprehended at the border. None of this counts the Godaways. If you add the numbers up, it's almost 7,000,000 people who have been encountered at the border just since president Biden took office, and at least 2,000,000 gotaways. This is Twice the population in my State of Louisiana. Fentanyl is the leading cause of death for Americans aged 18 to 49 in this country. Fentanyl poisoning, because it's allowed over the border, we have human trafficking and all the other terrible things. In the last 3 months, October November, December alone, we've had more illegal crossings at the border than in any entire year the end during the Obama administration. The American people see this. They feel it acutely. They see all the terrible societal ills that come from this, and it must be addressed. So I've made this very clear again from the very beginning When I was handed the gavel, we needed clarity on what we're doing in Ukraine and how we'll have proper oversight of the spending of precious taxpayer dollars and the American citizens

So a war that cost billions of dollars, hundreds of thousands of lives, CIA operation, covert action, links with Libya, never explained, never discussed. And even when the government falls last week, no background given. You know, we're supposed to have amnesia. We're not supposed to understand that what happens is the result of long term plans that have been pretty disastrous. And by the way, as I've said, Israel has driven so many American wars, and we say, absolutely yes. That's our greatest ally. These have been at huge cost to The United States, cost of trillions of dollars, cost geopolitically, but somehow we gave away our foreign policy to Israel years and years ago, and it's been absolutely devastating. And it's interesting to go back and watch Netanyahu speak to the American people. Go look at a video clip of 02/2001, '2 thousand '2. In 02/2002 in October, he comes and testifies in the senate, and there's a nice clip of him promising how wonderful the war in Iraq is gonna be, because, Saddam Hussein has weapons of mass destruction. He says a % certain. Complete lies, by the way, and they knew that they were lies at the time. And it's gonna be wonderful. We're gonna topple that dictator, and then dictators are gonna be toppled everywhere, and the young people of Iran are gonna rise up. This is his idea together with his US Political Consultants, together with neocons in the US government for the last twenty five years. They have never apologized for dragging The United States into countless wars in The Middle East, spending trillions of dollars running up US debt, and doing what? Creating chaos. So just to go back to the seven countries, because it's worth remembering. Lebanon, it barely exists as a functioning country right now. Syria, it's going to be picked to pieces. Don't believe Well, it's obvious in what we're seeing every day. Territorial integrity? Yeah. Israel's just invaded from the Southwest into deeper Syria, Turkey from the North. Russia has its area, The United States and the Kurds have their area. This place is just gonna be a battlefield for years to come. Iraq. We know what happened with Iraq. Trillions Of Dollars, a complete destabilization of the country. Look at the other three wars. The United States broke apart Sudan. Why? Well, Sudan was an enemy of Israel, so we have to break apart Sudan. So we supported the South Sudanese. Now we have the the real trifecta, massive civil war in Sudan and massive civil war in South Sudan. In other words, we broke apart the country, and now there's civil war in both halves of the country. Somalia basically doesn't exist as a country. Libya, it doesn't exist. It's a battlefield. It's a war zone. So that's six out of six. And Netanyahu's crowing. Now we go on to Iran. Come from? We've paid probably $7,000,000,000,000 if you add it up according to Brown University studies, for example. Something like $7,000,000,000,000 has gone into this. Israel couldn't do this for one day. Israel, you know, Netanyahu, we are lions. Yeah. Right. You are liars, but, we are the ones funding you, arming you, paying for all of this. That's The United States. And this is weird to me because we say yes to defend our ally. No. No. No. We're doing their foreign policy, which makes no sense, which doesn't lead to any peace, which leads to basically a war zone across the Middle East, and we say this is good for us. Why is this good for us? What what do what's The United States getting out of any of this? We haven't gotten anything out of any of this except massive geopolitical isolation. The most recent votes in the UN, for example, put The United States alone, alone with Israel. And I shouldn't exaggerate. We have Micronesia on our side. We have Nauru on our side with its 12,000 people. Maybe a couple of other countries. The whole rest of the world is saying, what is going on? Endless war in The Middle East. Well, this is because we're defending someone with some seventh century BC vision of what they want their country to be. Were Americans involved in the overthrow of Assad last week? Of course, they were, because this has been an ongoing operation. Whether they were involved in the final days, I don't know. They were involved in the in the thirteen years nonstop. I don't understand how. Actually, let me tell you an interesting story, by the way. The war started in 2011. Was portrayed as always as the CIA does as a local uprising and the freedom fighters, and it was said this was Syrians protesting against Syria. That's always how any CIA regime change operation works. There may also be local opposition, but the CIA is the it provides the armaments. It provides the flow of heavy weapons. It provides the financing. It provides the training. It provides the camps. It provides the political organization. So this started in 02/2011.
